PERMISO PARA “THE JOURNEY LOCK-IN” (para jóvenes de 12 a 18 años) (YOUTH LOCK-IN Permission Slip).

Por la presente doy mi permiso para que mi hijo/a (I hereby grant my permission for):

Al firmar este permiso, entiendo lo siguiente:

* Todos los jóvenes deben tener entre 12 y 18 años.
* Los padres deben proporcionar un número de teléfono donde se les pueda localizar durante la noche en caso de emergencia.
* Se llamará a los padres para que recojan a su adolescente si el adolescente abandona el edificio por un período de tiempo prolongado y/o actúa de una manera considerada inaceptable por los líderes del Ministerio Juvenil de CFC “The Journey”..

By signing this permission slip, I understand the following:

* All youth must be 12 to 18 years old.
* Parents must provide a phone number where they can be reached during the evening in case of emergency.
* Parents will be called to pick up their teen if the teen leaves the building for any length of time and/or acts in a manner deemed unacceptable by “CFC The Journey Youth Ministry Leaders):

REGLAS (LOCK-IN RULES):

- Todo joven deberá firmar esta hoja, indicando su conformidad con estas normas.
- Nadie puede asistir al “Lock-In” sin un permiso firmado por los padres y números de contacto. Se llamará a los padres para verificar si hay dudas.
- A los participantes no se les permitirá salir de la iglesia o del lugar sin la supervisión de un adulto.
- No se permiten insultos ni sarcasmo: No menospreciamos a los demás, no nos burlamos de los demás (especialmente de aquellos que no están con nosotros), no molestamos a los demás ni chismeamos.
- Se respetuoso: No usar ropa reveladora, no mostrar muestras de afecto, no perseguir, tirar o lastimar a otros.
- Mantenlo limpio: ¡sin malas palabras!
- Durante el Lock-In, todos los jóvenes deben permanecer dentro de la iglesia. No se permitirá ninguna salida o entrada no autorizada.
- NO se permiten videojuegos personales ni tarjetas de colección.
- Si el joven necesita tomar alguna medicina, los padres deberán entregarla a los lideres con las indicaciones claras de como y cuando tomarla.
- No se permiten drogas ilegales, alcohol, materiales peligrosos ni armas de fuego.
- Todos los participantes deben respetar los límites físicos de los demás.
- Se proporcionarán dormitorios para personas del mismo sexo.
- Se espera que todos los participantes (jóvenes y adultos) participen en el programa.
- Los participantes deben cuidar y limpiar lo que se use.
- Ser considerado y respetuoso con los demás participantes y líderes.
- Ser respetuoso con la iglesia y la propiedad de la iglesia.
EL CUMPLIR CON LAS REGLAS DEL “LOCK-IN” DE ES RESPONSABILIDAD DE TODOS.

- All youth must sign this sheet, indicating their agreement to these rules.
- No one is allowed to attend the lock-in without a signed parental permission slip and contact numbers. Parents may be called to verify if there is doubt.
- Lock-in participants will not be allowed to go outside of church or venue without adult supervision.
- No insults or sarcasm: We do not put others down, make fun of others (especially those who aren’t with us), pick on others, or gossip about.
- Use your body respectfully: No revealing clothing, no PDA, no chasing, throwing, or hurting others.
- Keep it clean: no swearing!
- During Lock-Ins, all youth must remain within the church. No unauthorized exiting or entering will occur.
- NO personal video games, trading cards
- Special medication or required items should be indicated to the leaders.
- No illegal drugs, alcohol, dangerous materials, or firearms.
- All lock-in participants must respect others’ physical boundaries.
- Same sex sleeping quarters will be provided.
- It is expected that all lock-in participants (youth and adults) will participate in the program.
- Take care of yourself and clean up after yourself.
- Be considerate and respectful of other participants and leaders.
- Be respectful of the church and property of the church.

THE ENFORCEMENT OF THESE LOCK-IN RULES IS EVERYONEʼS RESPONSIBILITY.

- Personal items (toothbrush, toothpaste, brush, deodorant etc.)
- Bathing suits and towel. (If girls are planning to wear a t-shirt on top of their bathing suit to get into the pool, the t-shirt needs to be white.
